1. How can I get a copy of my DD214 (discharge)?

Standard Form 180, Request Pertaining to Military Records is available at your local Veterans Assistance office and on-line.

2. Am I eligible for VA health care?

Veteran status is established by length of active duty service in the military and a discharge or release from active military service under other than dishonorable conditions. For information call 1888-878-6889 and ask for eligibility department.

3. I am a woman veteran, am I eligible for any benefits?

Women veterans are eligible for the same VA benefits as male veterans.

4. Can I get a VA loan for a home or manufactured home?

You will need a Certificate of Eligibility to participate in the VA home loan program. You can apply by submitting  VA Form 26-1880 along with proof of honorable military service (DD214). If you are still serving regular active duty, you must include an original statement of service signed by the adjutant, personnel officer, or commander of your unit. If you were discharged from the Selected Reserves or the National Guard, you must include documentation of at least 6 years of honorable service. If you are still serving in the Selected Reserves or the National Guard, you must include an original statement of service signed by the adjutant, personnel officer or commander of your unit and again, at least 6 years of honorable service must be documented.

5. Is the surviving spouse of a deceased veteran eligible for the home loan benefit?

The unmarried surviving spouse of a veteran who died on active duty or as the result of a service-connected disability is eligible.

6. Can I get a VA disability pension?

Veterans with low incomes who are permanently and totally disabled may be eligible. You must have 90 days or more active military service with at least one day of which was during a period of war, be discharged under conditions other than dishonorable. Veterans of a period of war who are age 65 or older and meet service and income requirements are also eligible to receive a pension regardless of physical conss of physical condition.

7. Am I eligible for a Government Headstone in a private cemetery?

Any veteran, Army, Navy, Airforce, Marines and Coast Guard discharged under conditions other than dishonorable are eligible. Also any member of  the Armed Forces of the United States who dies on active duty.

8. What is the Presidential Memorial Certificate?

Certificates signed by the president are issued upon request to recognize the military service of honorably discharged deceased veterans. Next of kin, other relatives and friends may request the Certificates in person at any VA Regional Office or by mail: Presidential Memorial Certificates(402E), National Cemetery Administration, 810 Vermont Ave. NW, Washington, DC 20420-0001. The request should include a copy of the veteran's discharge document and copy of death certificate, and clearly state what address the certificate should be sent.

9. What can the local Veteran's Assistance Office do for me?

The local VA office supplies various Federal and State forms such as pension, service connected disability, education for dependents, G.I. Bill, home loans, headstones and burial flag, surviving spouse benefits, disabled license plates, and medical care. They also assist with filling out the forms and will help you file the forms through the proper channels.

10. How can I get to a medical appointment at the Veterans Hospital if I am unable to drive?

There is a DAV van that transports veterans to medical appointments from Terre Haute to Indianapolis VA Hospital. The van runs Monday, Wednesday, and Thursday. For more information call 812-462-3261.
